The required forms must be filled out and handed in on the first day of camp for all camp participants:
Please scroll down and click on the links to download the four required forms. The optional form is the Pharm4Me release form which would allow WNE to share camper contact information with Pharm4Me to allow the camper to receive information from this organization in the future.
For all campers, a parent or guardian must be present to sign all the required forms on the first day before camp starts. Please consider filling out the required forms ahead of time to reduce the long line when dropping off. We will be at the front doors of the Center for the Sciences and Pharmacy (1215 Wilbraham Road, entrance facing Wilbraham Road) to welcome you.
We encourage all campers and parents/guardians to review the Golden Bear Summer Camp Manual below before attending camp.
