Background

Kam L. Capoccia received her B.S. degree in Pharmacy from the University of Connecticut and her Pharm.D. from the University of Colorado Health Sciences Center in Denver. She completed a general practice residency at the University of North Carolina Hospitals in Chapel Hill and a specialty residency in Primary Care at the University of Washington Family Medical Center in Seattle. Dr. Capoccia has served as faculty at the University of Washington School of Pharmacy and the University of Washington School of Medicine. She has also taught at the MEDEX Northwest Physician Assistant Program.

Dr. Capoccia has spent more than 20 years working in collaborative practice agreements. Her practice focuses on chronic disease state management such as diabetes, hypertension, dyslipidemia, anticoagulation, and smoking cessation. She has been a Board Certified Pharmacotherapy Specialist (BCPS) since 2000 and a Certified Diabetes Care and Education Specialist (CDCES) since 2015.

Interests

Diabetes Management

As a Certified Diabetes Care and Education Specialist (CDCES), I have a passion and love for all things diabetes. I have the privilege and honor of working with people with diabetes and conducting research related to diabetes management. I volunteer for the Association of Diabetes Care and Education Specialists and I am a mentor for the Certification Board for Diabetes Care and Education. As the director of an accredited Diabetes Self Management Education and Support program, I provide patient care and education at the Western New England University College of Pharmacy and Health Sciences and Big Y Foods, Inc. Consultation and Wellness Center in Springfield. We are located in the Big Y on Cooley Street and serve the greater Springfield community.
